Outreach Counselor Department: Counseling Location: Melrose Park Office.

  • Salary: $45,00 – $48,000 (Depending on qualifications)

Position Summary

Under the supervision of the Program Manager, the Outreach Counselor provides street outreach and counseling services to high-risk adolescents. This role focuses on engaging youth and families, conducting assessments, delivering counseling and life skills support, and connecting participants to appropriate internal and external resources. The Outreach Counselor works closely with community partners and YOS programs to support youth development in diverse community settings while maintaining accurate documentation and strict confidentiality standards. Outreach Counselor Key Responsibilities: Conduct outreach to engage high-risk youth and their families in services. Meet with parents/guardians to support youth engagement. Provide counseling, employment, educational, and life skills training. Coordinate with YOS services to connect youth and families to resources. Offer transportation to youth as needed. Build relationships with referral sources that are beneficial to clients. Make referrals to appropriate internal and external programs, ensuring timely follow-ups. Develop and implement service plans tailored to youth needs. Maintain accurate documentation in electronic records. Participate in team meetings, supervision, and collaborative agency programs. Outreach Counselor Qualifications: Bachelor’s degree in Social Work, Counseling, or Psychology (required). Experience working with at-risk youth and families. Familiarity with outreach programs. Valid driver’s license and reliable transportation with proof of insurance.
