Treasure Coast Community Health (TCCH), a long-established and highly respected healthcare provider and Federally Qualified Health Center with locations throughout Indian River County, FL is seeking a Care Gap Quality Trainer to join our team.
The Care Gap Quality Trainer serves a dual role focused on improving patient care outcomes and operational efficiency. This position is responsible for identifying and addressing care gaps for Medicaid Managed Medical Assistance (MMA), Private Insurances and/or Medicare patients while also supporting staff training on Electronic Health Record (EHR) systems and clinical workflows. The role ensures that patients receive timely and appropriate healthcare services and that healthcare personnel are well-trained on systems and procedures that improve quality and compliance.
Key Responsibilities
- Review and filter care gap lists received from any insurance providers as directed by Quality Manager.
- Log into the insurance portal(s) to identify specific care gaps for assigned patients.
- Schedule physicals and preventive care appointments as needed.
- Ensure documentation is up-to-date and submitted to insurance providers appropriately.
- Verify insurance eligibility and update electronic health record notes.
- Notify patients of outstanding care needs or missing documentation.
- Maintain confidentiality and data accuracy in all records.
- Develop and deliver training sessions for new and current employees on clinical systems (EHR/PM) based on quality measures and clinical outcomes.
- Create and maintain clinically accurate training materials, exercises, and manuals.
- Conduct needs assessments, evaluations, and competency assessments for staff.
- Maintain and update electronic training environments.
- Assist in testing new system functionality or upgrades.
- Support "super users" and end-users with troubleshooting and optimization.
- Participate in long-term training planning and "train-the-trainer" programs.
- Provide clear and timely communication to staff, management, and patients.

Education and Experience
High school diploma or equivalent, minimum five years medical assisting experience and CPR is required. One-year certificate from college or technical school OR 3–6 months related experience/training in healthcare or EHR training, two to four years of experience in a primary care setting or clinical training environment, knowledge of medical terminology is preferred. Experience with healthcare systems and patient care processes is highly valued.
