We are seeking a Targeted Case Manager to support students within a school district setting, helping them navigate behavioral health needs, access essential services, and achieve success both academically and personally. In this role, you will collaborate closely with educators, clinicians, and community partners to develop and coordinate individualized care plans, advocate for student well-being, and promote positive outcomes in a supportive, team-driven environment.

- Functions/Responsibilities: Coordinating the development of effective service plans through optimizing the recipient’s personal and community resources Performing a comprehensive assessment and periodic reassessments of the recipient's needs for medical, educational, social, or other services Promoting multi-disciplinary care that is patient-centered and considers all aspects of recipient’s personal, psychological, economic, and cultural needs. Monitor and track service delivery Providing advocacy consistent with the recipients need and or barriers to service Maintaining thorough knowledge of CFMC services/benefits, public/private community resources, and federal/state laws which affect service planning Completing required documentation of services and plan of care per policy

- QualificationsMinimum
- Qualifications: One (1) year of TCM experience providing services to the identified population Have a Bachelor of Arts or science degree in behavioral health science such as psychology, sociology, social work, family studies, human studies, counseling, nursing, behavioral analysis, public health, special education, gerontology, recreational therapy, education, occupational therapy, physical therapy, speech language pathology, rehabilitation counseling, or faith-based education OR be a certified alcohol and drug counselor with any Bachelor of Arts or science degree AND Complete required case management training i.e. 12 hours of core training with an additional 6 hours of training consistent with the population group served, i.e. children with a severe emotional disability, individuals with a severe mental illness, individuals with a substance use disorder and/or individual with complex physical health issues paired with a substance use or mental health disorder
Preferred Qualifications
Five (5) years of experience providing targeted case management services to the identified population Demonstrated ability to provide leadership Advanced knowledge of local, state and national resources related to service planning Additional Requirements: Ability to work effectively in a team environment Work hours are congruent with the needs/requirements of the position Provide transportation to individuals/ family members(s) served through TCM services Schedule Requirements: Full-time position, 5 days per week, 100% clinic-based, in person work required Work schedules are set at the discretion of CFMC and may be adjusted at any time based on the operational needs of the organization.
