  • POSITION SPECIFICS Title: Mental Health Therapist/Clinical Psychologist FTE: Full Time (40 hours/week) Schedule: Monday-Friday 8:00am-4:30pm Holiday Rotation: None Weekend Rotation: None On Call Requirements: None POSITION SUMMARY The Mental Health Therapist/Clinical Psychologist functions as an independent healthcare provider who, working collaboratively within a multidisciplinary team, provides diagnostic assessment and treatment to applicable demographic population to address the needs of these patients and provide clinical mental health services.
  • Responsibilities include assessment/intake, treatment planning, individual clinical therapy, and related documentation.
  • Emphasis is placed on the provision of evidence-based clinical behavioral health practices.
  • These services may be provided in a variety of settings, dependent on the population served (i.e. clinic settings, via telehealth, school settings, senior living facility).
  • In addition, this role may also perform duties associated with the Behavioral Health department in connection with the clinics/hospital as needed.
  • POSITION TECHNICAL RESPONSIBILITIES Perform assessments to evaluate a variety of mental health and behavioral conditions.
  • Collaborate and consult with a multidisciplinary team to address the needs of the population served, provide quality patient care, and to promote SPH’s vision and mission.
  • Coordinate patient care with a variety of resources.
  • Resources will vary based on specific population served and could include, but is not limited to: schools, families, primary care, senior living facilities, and other specialty providers as needed.
  • Conduct individual/family/group counseling, utilizing evidence-based practices.
  • Utilize outcome tools to determine patient progress.
  • Assist in contributing to continuity of care planning as patients transition to other community-based services.
  • Maintain progress notes and other required electronic documentation.
  • Serve as a subject matter expert as part of a multidisciplinary team, provide feedback, and consult as needed.
  • Work collaboratively with Sauk Prairie Healthcare on projects and initiatives related to Behavioral Health Services.
  • Provide care within their scope of practice as outlined by applicable state law, licensing, regulations, SPH policy, and practice agreements.
  • POSITION REQUIREMENTS Education: Mental Health Therapist Required: Master’s degree in Social Work, Psychology, or Counseling.
