SUMMARY

Under the general direction of the Program Manager, the Case Manager provides education and social service support to individuals and families through home and/or community visitation. The Case Manager assesses the needs and develops and implements a program plan. Responsibilities include home visitation, education and support with health, nutrition, finances, parenting, early childhood development, referrals to other community agencies, follow-up, and data tracking. Develops a positive relationship with children, parents, and families.
