- Coordinator include, but are not limited to: Completing Intakes, Coordinating eligibility determination process Assisting families in locating service providers Collaborates with team members to develop individual family service plans (IFSP) Maintaining early intervention records Maintaining up to date accurate information on Cornerstone system Maintaining constant contact with families, Facilitating six-month IFSP reviews, IFSP updates and Annual IFSPs Implementing transition plan Completing the family fee schedule Conducting developmental screenings and public awareness presentations
