Post Secondary Transition Counselor

The counselor will prepare students for adulthood by combining counseling, program development, and family collaboration, focusing on building student independence, life skills, and confidence. Responsibilities include applying therapeutic frameworks, developing individualized transition plans, and leading multidisciplinary meetings to align objectives.

Candidates must possess a master's degree in a relevant field (Counseling, psychology, social work) and hold a valid connecticut endorsement such as 068, 070, or 071. Strong experience supporting students with learning or emotional challenges and excellent collaboration skills are also required.
