• SUMMARY: Organizes, plans, and administers medically prescribed speech therapy programs in the hospital and outpatient clinic to assess and treat people who have speech, language, voice, and/or fluency disorders. Understands and provides age-appropriate care to all ages from pediatric, adolescent, adult and for geriatric patients.
• PRIMARY
• Responsibilities &
• AUTHORITIES: Evaluates, records and reports on patient’s progress for review by other members of the rehabilitative team and to the referral source that provided the patient for treatment. Designs and implements individualized treatment plans to address functional needs for each patient. Maintain open communication with the Rehabilitation Team regarding patient’s performance/progress. Initiates and maintains communication with interdisciplinary team regarding patient’s progress, needs, and discharge planning. Exhibit positive attitude and behaviors facilitating team work, efficiency, and care quality and promoting a favorable unit and company image. Conducts or participates in training medical and nursing personnel and other workers in speech therapy techniques and objectives. Must attend all mandatory in-services. Promotes and participates in performance improvement in-services. Responsible for filling out appropriate patient billing forms and treatment notes daily.

• MINIMUM

• ABILITIES): Graduate of an accredited school of speech/language pathology Must be willing to work with all age ranges from Pediatrics to Geriatrics.

Position

• Speech Language Pathologist OTHER SPECIAL REQUIREMENTS (LICENSES, CERTIFICATIONS, REGISTRATIONS, ETC.) Licensed with the State of Kentucky to practice Speech Therapy with at least a Master’s Degree from an accredited school in Speech Language Pathology and maintain eligibility for license renewal.
