About the Role

  • Come join our team as a full-time Certified Diabetes Educator with BJC Medical Group located Saint Louis, Missouri.
  • This is a rare opportunity to join a great team.
  • Outpatient Preferred experience with Diabetes Management Preferred Certified Diabetes Education or ability to certify 40 hours per week Days - Monday to Friday 8:00am-4:00pm No weekends or holidays!
  • Primarily at Christian Hospital, but shared with IL offices as well.
  • Travel will be required a minimum of 1-2 times per week Must be license in MO & IL

Preferred Qualifications

Role Purpose Provides Diabetes and Nutritional educational services and community resources for inpatients and outpatients. Maintains a case load of patients and promotes program development.

Responsibilities

  • Monitors patients response and modifies plan of care accordingly. Provides accurate documentation as required. Delivers direct patient care by assessing and evaluating patient's physical, psychosocial and emotional needs. Develops and implements individual plans of care in accordance with the established guidelines and standards of nursing care. Educates patients and their families on how to manage their condition.
