Core responsibilities
The intensive care coordinator provides community-Based trauma-Informed services and case management to support transition-Aged youth in achieving mental health, housing, and educational goals. They facilitate child and family teams to connect youth with natural supports and formal system partners to address comprehensive needs.
Requirements summary
Candidates should have at least two years of experience working with system-Involved youth and possess direct lived experience with foster care, social services, or related community resources. A bachelor's degree in a related field is preferred, along with the ability to travel by car for client meetings.
