Community Health Worker - Mission Plaza

The community health worker will develop and deliver health education programs, conduct outreach, and provide care coordination for clients. They are also responsible for maintaining project databases, managing calendars, and promoting health initiatives through social media.

Candidates must have a community health worker certification and either a relevant degree with two years of experience or five years of experience in health education. Proficiency in english and a second language such as spanish, arabic, or pashto is preferred.
