Job description

Position Overview Job Coach Provides training and ongoing support to clients to help them succeed and maintain employment. This includes supervision, skill-building, and support at job sites. Adult Day Services Assistant Supports the daily operations of an adult day program by creating a safe, structured, and engaging environment. Responsibilities include leading activities, providing supervision, and encouraging social interaction.

Combined Responsibilities Program Activities & Engagement Plan, lead, and supervise group activities such as arts & crafts, music, games, exercise, wellness, education, and community outings. Encourage participation, social interaction, and skill development. Help maintain the daily schedule and assist with smooth transitions between activities.

  • Client Employment Support (Job Coach Duties) Train clients and supervise work performance.
  • Document client progress with regular case notes (at least monthly).
  • Communicate with site supervisors to ensure employer expectations are met.
  • Advocate for clients and act as a liaison between clients and employers.
  • Support independence through fading strategies.
  • Follow behavior management plans as needed.
  • Respond to conflicts, behavioral issues, and medical needs.
  • Assist with pre-employment skill development as directed.
  • Model appropriate workplace behavior.
  • Recommend workplace accommodations when appropriate.
  • Transport clients to and from job sites using company vehicles.
  • Complete additional duties as assigned.

Participant Support & Care Provide respectful, person-centered support and help individuals achieve personal goals. Develop and implement individualized support programs. Assist with personal care (feeding, toileting, daily living tasks) as needed. Monitor health needs, diet, and overall well-being.

Documentation & Reporting Maintain accurate and detailed case notes. Track attendance, participation, and behavior. Complete incident reports as required.

Safety & Environment Maintain a clean, organized, and safe environment. Provide consistent supervision to ensure participant safety.

  • Required Skills & Qualifications Ability to assist clients physically and use assistive devices.
  • Ability to provide personal care when needed.
  • Positive attitude and ability to motivate others.
  • Strong communication and instructional skills.
  • Ability to build professional relationships with staff, clients, and the public.
  • Basic computer skills (Microsoft Office, internet use).
  • Ability to communicate effectively with individuals with disabilities.
  • Ability to pass a Caregiver Background Check.
  • American Sign Language (ASL) is a plus but not required.
  • Valid Wisconsin driver’s license and ability to pass a DMV check.

Preferred Qualifications

Background in Human Services, Occupational Therapy, or Personal Care. At least one year of experience supporting individuals with disabilities. Willingness to learn American Sign Language (ASL).
