Care Manager

Care managers provide outreach, enrollment, and assessment services for children in the new york state children’s health home program. They develop comprehensive plans of health care and coordinate with families and providers to ensure the execution of these plans.

Candidates must have a bachelor's degree and at least 2 years of relevant experience in child welfare, behavioral health, or social services. Strong communication skills and the ability to work with diverse populations are required, with spanish fluency being a plus.
