Job description

Primary Function

  • Responsible for supporting individuals with Autism and Developmental Disabilities in development of skills needed for greater independent functioning and community integration. Support will be provided to the individuals in the community in social and vocational activities as well as classroom-type settings.

  • Include: Prepare and lead meaningful groups to facilitate members personal, social, vocational and independent growth Assist members with following their daily activity schedule Develop and maintain relationships with community organizations, programs, businesses and employers to ensure ongoing connections for community inclusion and learning opportunities Work with Site Supervisor and plan recreational and activities of daily living outings Drive up to a 10-passenger van to transport members to and from program and/or community activities as needed Supervise members on community activities and ensure their safety Attend and participate in supervision sessions with Site Supervisor or Program Coordinator as scheduled Document program member's progress towards ISP goals and objectives according to DDD and Medicaid guidelines Work with Site Supervisor on review of members' progress and recommendations for new goals and objectives Other duties as assigned by Site Supervisor, Program Coordinator or Director of Day and Community Services Completes all assigned College of Direct Support and Relias trainings as assigned Attends all scheduled meetings as assigned Meets relevant continuing educational/training requirement Completes all required documentation/paperwork accurately and within established time frames

Requirements
