  • Summary: Our Supported Employment Job Coach provides training and supervision for developmentally disabled adults in the Community Integrated and/or Supported Employment Programs. The Job Coach acts as a liaison between community-based employers, specific individuals receiving services, and necessary Mountain Valley staff. The Job Coach assists in developing, implementing, and monitoring client programming and in monitoring and responding to the client’s job satisfaction.

  • Who are we: Our Mission at Mountain Valley Developmental Services is to encourage and support individuals with developmental disabilities, enhance their ability to live, learn, and work in an independent and inclusive manner, and to educate the community about their contributions and capabilities. MVDS supports adults and children in Eagle, Garfield, Lake and Pitkin counties with intellectual and developmental disabilities (I/DD). Our work provides life span services for these individuals, their parents and family members, guardians, colleagues at work, neighbors, and friends to promote independence, quality of life, and rights protection. Responsibilities

  • Mentor and support individuals in achieving their personal goals and promoting independence. Treat clients with respect and dignity. With Employment Coordinator's assistance, develop task analysis on all parts of job as needed. Provide hands-on training and contact with clients at the job site and be able to facilitate training as needed. Maintain an awareness of problem areas during training and report these to the Employment Coordinator for assistance as needed, i.e. safety issues, barriers or accessibility problems, revision of task analysis. Act as a role model for the client at the work site. Arrange transportation to and from work site for clients. Assist clients in developing and maintaining independent transportation skills necessary for work. Communicate client concerns to Employment Coordinator and other appropriate staff and families. Track and submit data to CIE Program Coordinator for reports for funding, monitoring, etc. Implement goals and keep accurate documentation of goal progress and notify appropriate staff of any goal changes. Maintain regular contact with employers to assure the placement site is successful for the client and the employer. Assure that the quality and quantity of work performed meets the employer's needs. QualificationsThe Job Coach is responsible for providing training and supervision for developmentally disabled adults in the Community Integrated and/or Supported Employment Programs. The Job Coach assists in developing, implementing, and monitoring client programming as well as in monitoring and responding to the client’s job satisfaction. Essential Functions Programming Each Direct Support Professional (DSP) will be assigned to one or more functional areas including but not limited to day services, residential services, greenhouse or recreation. Each DSP is responsible for completing any tasks required in his or her assignment in addition to those functions enumerated below. With Employment Coordinator's assistance, develop task analysis on all parts of job as needed. Provide hands-on training and contact with client at the job site and be able to fade training as needed. Maintain an awareness of problem areas during training and report these to the Employment Coordinator for assistance as needed, i.e. safety issues, barriers or accessibility problems, revision of task analysis. Keep accurate documentation of goal progress and notify appropriate staff of any goal changes. Maintain regular contact with employers to assure the placement site is successful for the client as well as the employer and provide ongoing support. Assure that quality and quantity of work performed meets the employer's needs. Attend all IP meetings as requested, and complete and compile all documentation required for IP meetings on time. Attend all department staff meetings and other meetings as required. Individual Rights – Protecting and respecting the rights of individuals served Respect each individual’s rights within the program and treat all individuals with dignity and respect at all times. Respond as a Mandatory Reporter. Report all incidents of suspected client rights violations, mistreatment, abuse, neglect or exploitation immediately according to Colorado state law and MVDS policies and procedures. Respect the values, opinions, preferences and choices of individuals. Person Centered – Focusing on what is important to an individual receiving services Educate and encourage individuals to make informed choices regarding their lives. Provide opportunities and choices for growth and development based on individual interests, preferences and input. Ensure access to community resources based on individual interests, preferences and input. Encourage residents to take ownership in maintaining their environment. Provide emotional support, social guidance and positive role modeling. Safety (Risk Management) – Protecting individuals from harm Assist individuals to maintain a safe and clean environment. Create and maintain program’s chore chart for a list of duties. Provide safe transportation of persons receiving services, drive defensively and follow traffic laws. Follow safety guidelines including, but not limited to healthy lifting, transferring, and back care. Identify and solve potential safety hazards, repair or maintenance needs or immediately report them to the Operations Manager. In case of emergency, follow the emergency plan identified for the facility; notify appropriate authorities and Mountain Valley staff as soon as possible. Conduct, document and participate in emergency drills as required. Ensure the vehicles are well-maintained and cared for. Interact successfully with people in times of crisis or unpleasant environments. Individualized Plan and Care Coordination Participate in the development of the Individualized Plan (IP) for each resident including analyzing progress and developing new goals for the biannual assessment. Provide services and implement goals in accordance with the Individualized Plan for each individual supported. Communicate with the Interdisciplinary Team as needs change. Assist individuals with physical and personal care as identified in the Individualized Plan (IP). Assist non-ambulatory individuals with physical transfer as necessary, including, but not limited to, pushing wheelchairs, assisting someone in standing from a seated or laying position and entering and exiting vehicles. Documentation Review necessary documentation at start of shift and enter necessary documentation at by the end of shift per MVDS policies and procedures. Respond to correspondence (emails, voicemails, Therap, etc.) in a timely manner. Medical Needs Communicate with the Medical Office about observed changes in physical and emotional needs in a timely manner. Accurately administer medications, document administration and report/document all medication administration errors, as directed by the Medical Office and medication administration policies. Follow health protocols and care plans. Ensure documentation of all health information as needed (vitals, seizure reports etc.). Other Required Duties Standards of Behavior/Conduct Respond to all requests for information or communication in a timely manner. Follow all policies within the MVDS Employee Handbook. Report all on-the-job injuries to immediate supervisor and Director of Human Resources within 24 hours of the occurrence and provide a detailed account of the events leading to the injury. Attendance and Scheduling Report for work on-time and as scheduled. Enter daily, accurate timekeeping records in timekeeping database. Attend all regular staff meetings and other meetings as requested by supervisory or administrative staff members. Work schedule as assigned. Have all time-off requests pre-approved. Have all over-time hours pre-approved or demonstrate they were due to an emergency. Work at any MVDS facility and be responsible for own transportation to work. Competencies Complete all mandatory new hire requirements and training as identified by the State of Colorado and Mountain Valley’s training requirements within 90 days of hire and maintain courses and certifications as required. Maintain skills with computers, take advantage of new technology, learn new tools quickly, use technology to enhance job performance and increase productivity. Maintain current understanding of technical processes/equipment. Adherence to policies and procedures Adhere to MVDS policies and procedures outlined in the policy manual, Employee Handbook, and other employee policies. Complete other duties and responsibilities as assigned to ensure effective functioning of Mountain Valley’s programming.
