Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. Our team is growing and looking for an experienced:

Speech Language Pathologist (Bilingual)

Shifts Available: PRN

Service Areas with Openings: Southeast & Southwest

Responsibilities

  • Facilitates rehabilitation of patients with emotional, physical, or mental disabilities.
  • Implements speech therapy care plans.
  • Coordinates care with entire team.
  • Educates patients and family members.
  • Prepares timely, complete, and accurate clinical notes.
  • Communicates effectively with patients, caregivers, healthcare providers, office support and other care team members.

Qualifications

  • Speech language pathologist in the State of Texas.
  • Home health experience highly preferred.
  • Bilingual (Fluent in Spanish) highly preferred.
  • Experience with Home Care Home Base EMR (preferred but not required).
  • Knowledge of home health regulations and general healthcare compliance.
  • Ability to handle physical workload.
  • Good driving record.
  • Proficient level experience in Microsoft Office (Word, Excel, PowerPoint, Outlook).
  • Exceptional communication and people skills.
