Job Description

Summary: This position is responsible for providing education and support to patients/consumers and their care partners via 1:1 interactions, creation of/teaching of classes and groups (in-person and virtual) and bedside education. Works with a team of others with daily Center operations (greeting visitors, assisting with set up for groups, responding to Center emails from consumers, assisting with phone calls for assistance, etc.). Team members include social worker/coordinator, contingent nurse(s), office specialist, contingent social worker as well as PT, OT, SLT staff who float in for classes and contracted individuals (art, music and massage therapists). Interacts/partners with other members of the larger hospital/system care team. Responsibilities And Duties: 75% Assesses learning needs, provides education, assesses learning achieved, and provides support and navigation to patients and families. 10% : Participates in quality control and assurance. 5% : Participates in process improvement activities. 5% : May participate in some aspects of clinical care as needed. 5% : Other duties may vary by department such as various program coordination and maintaining registries.
