Job description

  • Looking for fun, creative and flexible staff to provide community based support services for individuals with developmental disabilities of all ages.
  • Community Support Professionals work one on one with individuals on their caseload to implement unique services, to explore and build skills which support increased independence, inclusion, and community engagement.
  • In terms of work locations listed above, you have the option of choosing the towns you prefer with a flexible daytime work schedule.
  • This position can be full or part time in Cortland County.
  • This position requires that you have your own transportation.
  • Those working fewer than 10 scheduled hours per week will earn $17.25 per hour.

Responsibilities

Responsible for the health and safety of the individuals supported Provide individualized services to people with disabilities and their families which may include: respite for families, teaching, modeling, and supporting skill development in a variety of areas at home and in the community Participate as an active member of the person’s team providing ideas, input, and advocacy for the person related to his or her Staff Action Plan Follow objectives as described in the person’s Staff Action Plan and as discussed with supervisor Attend training to improve services provided Complete job requirements (training, PPD, etc) within appropriate time frames Report any potential incident to the designated Family Support Coordinator/Supervisor immediately, or the day that it occurred. Accurately enter time and services into Racker’s electronic health record, eVero, documenting service times, supports provided, and mileage according to program and regulatory requirements (e.g., contemporaneously documenting service provision) Use of smartphone for documentation purposes required. Ownership/usage of personal smartphone is strongly preferred Communicate regularly with supervisor regarding service provision Attend scheduled supervisory meetings regularly Uphold standards of professionalism with regard to behavior and dress Assume responsibility for related duties as required or assigned

Qualifications
