Community Health Worker

The community health worker provides peer support and role modeling to improve life skills and health/Wellness through personal interactions, including instructing on wellness and chronic disease self-Management. Essential functions involve conducting home visits, assisting with care plan implementation, connecting clients to community resources like food and shelter, and documenting all services and program data.

Required qualifications include a high school diploma or equivalent and successful completion of the paraprofessional chw training program within 180 days of hire, along with obtaining specific basic life support (Bls) certifications within 60-90 days. Candidates must possess reliable transportation, a state of michigan driver's license, and auto insurance upon hire, with an associate's degree or three years of relevant experience preferred.
