Core responsibilities
The coordinator assists individuals aged 14 through 22 and their support teams in transitioning into dd/Abd waiver system services by developing meaningful, person-Centered objectives and collaborating to find necessary resources. Essential duties include ensuring compliance with he-M regulations, attending iep meetings, developing individual service agreements, and monitoring established goals.
Requirements summary
Candidates must possess a bachelor's degree in human services or a related field, along with a minimum of two years of experience working with individuals with intellectual/Developmental disabilities. A valid driver's license, access to a safe vehicle, and specific automobile liability insurance coverage are mandatory requirements.
