Responsibilities
Nurse Practitioner FT position available. Monday – Friday, daytime hours. Exciting opportunity for an experienced NP to join a busy Family Medicine practice in Batavia, NY and become part of an interdisciplinary team that provides patient care utilizing the Patient Centered Medical Home model. This is a fast-paced, semi-rural, community-based practice in the Batavia, NY area with opportunities to work with a variety of patients including Spanish-speaking patients (Language Line interpreters available). The providers care for patients including newborns, children, teenagers, adults, and older adults across all ages and medical issues. Responsibilities include: evaluation and management of acute/chronic illnesses, ordering/interpreting/communicating diagnostic tests and results, complete physical exams, wellness visits, hospital/ED follow-ups and transitional care management, collaboration with network based physicians and medical team regarding patient care, facilitating appropriate referrals to specialists or community-based agencies, opportunities for office based procedures, and patient education/counseling intended to maintain or restore physical/emotional health and function, and educational support for various learners consistent with the goals of the UR teaching institution. The NP will establish a team supported patient panel. Excellent communication and collaboration skills are essential with the ability to exercise independent judgement and initiative. Greater than 2 years of active practice is required and an NP is favored. An APP mentoring program is available for all new hires along with thoughtful onboarding. The candidate must possess NYS NP certification along with licensure and have applied for DEA registration prior to hire/start date. Nurse Practitioner Require a Master's degree as a Nurse Practitioner, an active New York State Registered Nurse license in good standing, an active New York State Nurse Practitioner licensure in good standing and current national certification as an Nurse Practitioner. NPs will demonstrate clinical competence, superior verbal and written communication skills, interpersonal effectiveness and customer relation skills, computer competence and must have an active desire to learn. The APP will participate in the education of learners (students and new employees), using clinical learning experiences, knowledge sharing and skill training.
