- Our Quest Tailored Day Services Program: In this program, we serve adults with developmental disabilities in their homes or the community, focusing on post-secondary education, vocational interests, and other personal goals.
- Community Mentors travel to meet 1-3 individuals per day and provide one-on-one direct support.
- As a Community Mentor you will be responsible for: Provide individual support in the home or community setting as identified in the participant’s Individualized Personal Plan.
- Work closely with the participant’s support team to identify needs and develop plans.
- Navigate private and public resources to create opportunities for growth, develop skills, and meet specific goals outlined in the Individual Service Design and IPP.
- Travel with participants throughout the local community to attend social events, recreational activities, school, etc.
- Advocate for participants’ rights and individual choice in all settings and provide services in accordance with HCBS Final Rule guidelines.
- Complete required documentation, including daily notes, activities using Buildability digital platforms, and written reports of client progress or barriers encountered in meeting goals.
- Perform other duties as assigned to assure the efficiency of program services.

- Qualifications: Strong interpersonal skills and the ability to interact with individuals with disabilities, their family members, employers, and others in a positive and constructive manner. High school diploma or GED. Must pass a DOJ Live Scan fingerprinting background clearance. Valid California Driver’s License, automobile insurance, and reliable transportation.
- Willing to travel: up to 75%. Cultural awareness and adaptability.
Preferred Qualifications
Fluent in Spanish. Familiarity with the Los Angeles Metro system (MetroGo app, route planning, etc.). Experience with resource navigation (Transportation resources, accessibility resources, etc.). Crisis management skills. Prior experience supporting individuals with disabilities. If you don’t have experience, we will provide on-the-job training.
