Case Manager (TLP)

The case manager provides comprehensive assessment, supported housing, vocational, educational, and intensive case management services to assist 17 1/2-20-Year-Old youth in residential care toward independent community living. Responsibilities include coordinating service plans, advocating for residents, attending court hearings, maintaining documentation, and providing direct supervision to staff.

Required qualifications include a bachelor's degree in psychology, social work, or a related human service, along with an illinois child welfare license and cerap certification. Candidates must also possess strong crisis management skills, cultural sensitivity, excellent communication, and have a clean driving record with a reliable vehicle.
