Self-Directed Community Habilitation Instructor

The instructor provides one-On-One assistance to individuals in their homes and communities to enhance life skills, social interaction, and community participation, while also facilitating mentoring relationships. Responsibilities include assisting with identifying community resources and ensuring timely submission of all required documentation like timesheets and service notes.

A high school diploma is required, along with excellent organizational, verbal, written communication, and interpersonal skills. Candidates must possess their own transportation for travel between work locations and hold a valid nys driver's license.
