Wraparound Facilitator- $2000 Sign On Bonus

The facilitator provides community-Based services centered on systems of care and wraparound services for identified youth, ensuring services are family-Focused, individualized, and strengths-Focused. Key duties include collaborating with youth and families to develop comprehensive plans of care and crisis prevention plans, monitoring progress, and advocating for needs in the least restrictive setting.

Candidates must possess a bachelor's degree in a related field or a master's degree in a human service field, along with two years of experience coordinating care, facilitating team meetings, and working directly with youth and families. A valid driver's license and the ability to work varying hours, including evenings and weekends, are also required.
