Service Coordinator - Early Intervention

The service coordinator establishes relationships with families of young children with developmental disabilities to create and sustain services outlined in an individual family service plan. Core duties include scheduling meetings, making referrals, providing advocacy and education, and ensuring all necessary supports and services are established and maintained across the county.

Candidates must meet specific educational or experience criteria, such as two years of service coordination experience, or a combination of experience and an associate's or bachelor's degree in a health or human service field. A valid nys driver's license is required, and the nys early intervention coordination certification must be obtained within the first three months.
