Health Education Specialist

The health education specialist collaborates with patients and healthcare teams to identify educational needs and reduce barriers to care. They provide general health education, facilitate care coordination, and maintain documentation in accordance with organizational and regulatory standards.

Candidates must possess an associate's degree with at least two years of experience in community outreach or counseling, with a bachelor's degree preferred. Proficiency in english and spanish is required, along with bls certification and the ability to obtain further certifications in health education and chronic disease management.
