Apex CSI Case Manager

This school-Based case manager will manage cases related to mental health care delivery for assigned clients at designated school locations, providing resources to parents and school systems for individual patient needs. Responsibilities include providing social services to clients and families in telfair county schools, ensuring medical appointments and medication adherence, and maintaining detailed progress notes within the emr.

Minimum requirements include a high school diploma/Ged, a valid georgia driver’s license, effective verbal and written communication skills, strong interpersonal skills, and experience with microsoft 365 office products. A bachelor's degree in a related field and previous case management experience are preferred qualifications.
