Youth Case Manager (Mental Health Care Coordination)

The role involves working with youth aged 0-21 with mental health diagnoses and their caregivers to identify needs and goals focusing on education, mental health, somatic health, economic needs, and positive social development. Staff meet with families weekly/Bi-Weekly in the community or home and provide necessary transportation to ensure successful connections to community resources.

A minimum of a 4-Year degree is required, preferably in a related human service field, along with at least one year of direct experience working with youth under 18, which can include volunteer or internship time. Applicants must also possess a valid driver's license for the past five years and be able to reliably commute to hagerstown, md.
