- Lake Charles
- Lafayette
Position Summary
The Case Manager role is responsible for facilitating the Wraparound process for families served in the program in a timely and responsive manner. The Case Manager will coordinate and schedule Family Team Conferences to promote individualized and family goals.
- Essential Job Duties
- Maintain information regarding the environment of current services to ensure a flexible, responsive, culturally competent service delivery system,
- Coordinate and facilitate Family Team Conferences in order to achieve consensus among family and other participants in developing an appropriate plan based on the needs of the families and to ensure compliance with AMIkids procedures values and standards,
- Ensure services and treatment are delivered from a strength based framework and customized in order to meet individualized needs of consumers and are culturally competent,
- Authorization and monitoring of resources, including agency and community to meet client needs,
- Foster and maintain ongoing relationships with family members, providers, and identified resources in order to call on others to implement client plans,
- Plan and schedule meetings, conferences, and travel schedules in order to maintain staff schedules,
- Meet with families in their home environment or other approved location,
- Operates program vehicles as required in accordance with the transportation policy,
- Attend required training/professional development events and maintain appropriate certification(s) and license(s),
- Assist with special projects and other duties as assigned.
- Minimum Education, Training and Experience
- Bachelors’ degree in Psychology, Human Services, Social work or related field; Master’s preferred,
- Minimum one year related experience, working with families and children, Wraparound experience preferred
- Strong computer skills,
- Strong collaboration, negotiation and problem-solving skills,
- Obtain and maintain appropriate crisis intervention and physical restraint training and certification as defined by state requirements,
- Obtain and maintain current CPR and First Aid Certification by nationally recognized organization,
- Be able to complete trainings outside the program as needed.
