Chronic Disease Coach

The coach will facilitate delivery of evidence-Based health initiatives programs, such as parkinson’s program and livestrong at the y, ensuring fidelity to the curriculum. Responsibilities include delivering classes, encouraging group participation, cultivating participant commitment, and traveling to community locations for program delivery.

Candidates must demonstrate a commitment to the y's mission and possess strong skills in communication, organization, and relationship building, along with empathy for health seekers. Familiarity with motivational interviewing techniques is highly encouraged.
