Behavioral Health Provider (Triad Local Schools CHC)

The behavioral health provider applies social work methods for the diagnosis, prevention, and treatment of psychosocial dysfunctions for individuals, families, and groups, operating within a team-Based care model. Key duties include screening records, conducting assessments, setting treatment goals, providing interventions, and consulting with primary care providers.

Candidates must possess a graduate degree in social work, counseling, or a related field, along with specific ohio licensure (Lsw, lisw, lpc, lpcc, or supervisors thereof). Experience demonstrating expertise in behavioral health practices and knowledge of community health center regulations and accreditation processes is required.
