  • You’ll Do:
  • Coordinate
  • Personalized
  • Plans: Develop and implement individualized plans for each client, including Staff Action Plans, Individual Plans of Protective Oversight (IPOP), Personal Expenditure Plans, Money Management Assessments, and Levels of Supervision.
  • Lead
  • Interdisciplinary
  • Teams (IDT): Facilitate team meetings to ensure that clients receive quality active treatment and that their needs are met holistically.
  • Monitor
  • Progress: Track and evaluate the progress of clients’ goals and achievements, adjusting plans as necessary.
  • Ensure
  • Compliance: Maintain accurate, up-to-date electronic records and ensure all documentation is in compliance with OPWDD (Office for People With Developmental Disabilities) regulations.
  • Support
  • Independence: Assist clients in gaining independence with life skills, social interactions, and other personal goals.

  • Training
  • Programs: Help develop and implement training programs for staff and clients, monitoring progress and outcomes. QualificationsEducational Requirements Bachelor’s degree in a Human Services field (or related field) required.
  • Experience &
  • Skills:
  • 1+ years of relevant experience in a similar role preferred.
  • Proficiency in person-centered planning, instructional methods, and progress tracking. Strong organizational, communication, and interpersonal skills.
  • Valid New York State driver’s license is required.
