Intensive Care Coordinator - Bachelor's

Provide care planning and coordination of services for eligible youth under the age of 21 using the wraparound model. Facilitate care planning team meetings and assist caregivers in learning effective advocacy skills.

A bachelor’s degree in social work, psychology, or a related field is required, along with at least 1 year of direct experience working with children, young adults, and/Or families. A valid driver's license and access to a reliable vehicle are also necessary for this position.
