- Candidates must reside within NorthLakes' service area at the time of hire and maintain residency throughout employment.
- This position is working with patients in person as well as via telehealth services throughout NorthLakes Community Clinics.
- Position may be part-time or full-time.
- The Registered Dietitian performs specialized work in applying the knowledge of nutrition to the prevention and management of disease and improvement in overall health.
- Works as a team member with providers and other team members to develop care plans for individual patients.
- Provides group educational classes and develops educational materials for patients and staff.
- Essential Job Functions Essential Functions of the Job: [Other duties may be assigned.] Assess patients eating habits, including intake and nutritional content, as part of their overall health care Provide in-depth counseling (in-person and virtually) to individual patients about eating habits and how they impact overall health Assess health education needs and teach patients about food selection and nutrition labeling Provide customized nutrition information of individual patients Teach patients with diabetes how to plan their meals to balance their blood glucose levels Educate pediatric patients and parents on the 5-2-1-0 program Plans and administers therapeutic treatment by telephonic/ telehealth methodologies as indicated by patient needs and state guidelines.
- Appropriate use of PPE/infection control protocols when providing treatment to patients.
- Qualifications and Education Requirements Bachelor’s Degree or higher in nutrition or dietetics or related degree.
- Two or more years of experience in nutrition counseling, some of which must include working with diverse and underserved/underinsured populations.
- Experience with diabetes education preferred.
- Additional Skills Knowledge Considerable knowledge of clinical applications of nutrition, practices of nutrition and nutritional requirements in health and diseases, such as diabetes; Strong skills in counseling and educating patients; Ability to assist in development and implementation of care plans for patients.
- Preferred Skills To perform this job successfully, an individual should have knowledge of Electronic Health Record Database software and Word Processing software.
- Required Credentials, Certification, Licensure Within twelve months of hire, registration through the Commission on Dietetic Registration and certification by the State of Wisconsin, if not already certified, required.
