Care Coordinator\Educator - Edie Windsor Healthcare Center - Hampton Bays

The patient navigator/Educator provides lgbtq+ health equity training to hospital staff and coordinates care for lgbtq+ patients. They also conduct patient intakes, develop individualized care plans, and facilitate access to medical and support services.

Candidates must hold a bachelor's degree or higher in a health-Related field such as community health or social work. Strong interpersonal skills, cultural competency, and proficiency in microsoft office applications are required.
