Case Manager (SCan) – Violence Prevention and Intervention (VPI)

The case manager provides trauma-Informed, youth-Centered support to high-Risk individuals by developing personalized case plans and connecting them to essential resources. They also conduct proactive community outreach and maintain accurate documentation of progress and service delivery.

Candidates must hold a bachelor’s degree in social work or a related field and possess a valid driver’s license with access to a reliable vehicle. Strong communication, relationship-Building skills, and a commitment to working with youth are essential for this role.
