Diabetes Education Specialist II

The diabetes education specialist provides assessment, goal setting, and behavioral intervention to patients and caregivers to promote diabetes self-Management. They also manage patient care documentation, develop educational resources, and coordinate care with other healthcare providers.

Candidates must hold a bachelor's degree and possess a cdces or bc-Adm certification. A minimum of one year of specialized experience as a licensed dietician or nurse is required.
