Community Health Worker- School Based

The community health worker will engage patients and families in care planning while collaborating with school officials to coordinate health and social services. They are responsible for conducting intakes, providing health education, and maintaining accurate documentation in compliance with organizational and regulatory standards.

Candidates must possess a high school diploma or ged and a valid driver's license, with at least one year of experience in health services or public health. A michwa community health worker certification and bls certification are required or must be obtained within the first year of employment.
