Community Health Worker

The community health worker provides basic health information from a standard curriculum and assists patients in connecting with necessary health, social, and other required services as part of the outreach team. This role involves developing relationships with local resources, referring patients to services, and actively participating in and coordinating outreach events.

Candidates must possess intermediate written and verbal communication skills in english, along with computational and computer skills typically gained through a high school education. Essential technical skills include experience working with underserved, low-Income populations and the ability to learn and apply popular education methods for effective health education delivery.
