Job description

Responsible for ensuring provision of quality nutritional assessments and education to Center’s patients through individual and group educational activities. Assists Director of Population Health in the development of nutritional programs for Center.

Duties: And responsibilities:

1. Plan and provide individual and group nutrition counseling and education to Center patients.

2. Assess the dietary and health needs of patients, guide patients to make lifestyle changes utilizing motivational interviewing, provide Medical Nutrition Therapy for appropriate patients, promote nutrition through public speaking and community outreach programs, and keep abreast of the latest research in nutritional and food sciences.

3. Educate patient and family members on nutrition, diet, food selection and food preparation.

4. Consult with other Center staff (physicians, nurses, health educator, and counselors) concerning the patient' food habits and dietary needs and serve on inter-disciplinary team for chronic disease education and programs.

5. Provide in-services on nutritional issues to Center's staff. Assist the Health and Wellness Team in outreach activities.

6. Develop and implement innovative approaches to deliver nutritional services in person and virtually.

7. Assist with preparation of program and National Collaborative reports.

8. Serve as liaison with local and state agencies and professional nutrition associations offering community nutrition education programs.

9. Evaluate effectiveness of patient interventions and submit reports to the Health and Wellness Team.

10. Perform other related duties as assigned.

Job qualifications: Bachelor’s Degree in Nutrition or Dietetics

Registered Dietitian and Licensed in Texas required

Fluent in English and Spanish required

Knowledge and experience in nutrition counseling on dietary patterns

Experience in nutrition counseling and education with pediatric and adult populations preferred

Ability to plan and organize work with minimal supervision

Excellent written and verbal communication skills

Ability to work well with all staff and volunteers

Ability to travel to Centers and community outreach as needed
