Case Manager - SKYS

The case manager provides specialized support to youth and young adults in areas such as education, employment, and housing to foster independence. They are responsible for developing individualized service plans, maintaining accurate documentation, and building collaborative relationships with community partners.

Candidates must hold at least a bachelor's degree in a social services field and possess experience working with at-Risk youth or young adults. The role requires a valid driver's license, the ability to pass a background check, and the capacity to meet physical and safety-Sensitive driving standards.
