  • Baptist Health is hiring a Baptist Health Wellness Centers Referral Coordinator.
  • This is a full-time, in-person opportunity to join the Baptist Health Partnerships.
  • This position will rotate to several different location throughout the Jacksonville area.
  • The Coordinator will be responsible for (but not limited to): Performing Wellness Coaching sessions delivering quality health education guidance and administering baseline health screenings Accurately documenting wellness coaching session information and program participation data Confidently learning and working within multiple technology systems such as: Microsoft Office programs [Outlook, Word, Excel, PowerPoint, etc.], Epic EMR system, and various web-based platforms like Eventbrite, SECA, Converge Virtual Merchant, and Smartsheet Program coordination including the development, promotion, implementation and evaluation of health education events Facility operations and the daily tasks of opening/closing the office, maintaining an organized space & supply needs, greeting and providing assistance to all patrons, and preparing the Center for scheduled programming Assisting patrons with healthcare navigation questions and needs **Wellness Coach Certification required or completed within 90 days Education/Degree Required: Bachelor of Health Science Or Bachelor of Health Administration Or Bachelor of Science in Public Health Or Bachelor of Science in Exercise Or Bachelor of Science (non Nursing) Or Degree Preferred: Master of Arts/Science (MA/MS) Certifications: 1-2 years Experience coaching wellness needs 1-2 years Experience with evaluating wellness and health promotion programs 1-2 years Experience implementing wellness and health promotion programs 1-2 years Experience conducting health risk assessments If interested, apply today!
