  • PURPOSE: The Inpatient Diabetes Care Specialist is responsible for coordinating and optimizing diabetes education and management for hospitalized patients and staff. This role leads the implementation, oversight and management of the GlucoStabilizer program and other diabetes related initiatives to ensure safe, and effective glycemic management. The specialist collaborates with interdisciplinary teams to improve clinical outcomes, enhance patient safety, and support evidence-based diabetes care practices for the MercyOne Genesis Health System. ESSENTIAL FUNCTIONS 1. Knows, understands, incorporates, and demonstrates the Trinity Health and MercyOne Mission, Vision, and Values in behaviors, practices, and decisions. 2. Serves as subject matter expert on diabetes care management, including staff education, troubleshooting and workflow integration with diabetes technology. 3. Oversees administrative management/programming and daily operations of the GlucoStabilizer insulin dosing software, ensuring compliance with protocols and best practices. 4. Monitors patient glucose trends and provide recommendations for insulin adjustments in collaboration with providers. 5. Partners with nursing, pharmacy, physicians, and IT teams to ensure seamless diabetes care delivery. 6. Ensures diabetes protocols and policies are up to date with clinical guidelines, and professional consensus statements. 7. Audits glycemic control data and identify opportunities for quality improvement. 8. Ensures protocols and organizational policies meet regulatory standards and is up to date with evidence-based guidelines and consensus statements. 9. Develops and maintains standardized workflows, policies, and documentation related to diabetes. 10. Participates in performance improvement initiatives and interdisciplinary committees related to diabetes care. 11. Provides diabetes education to all newly hired and other clinical colleagues as needed via orientation or other educational platforms. 12. Provides diabetes education to inpatients newly diagnosed with diabetes as needed. 13. Performs other duties as needed and assigned by the manager. 14. Maintains professional relationships with vendors for updated information and available diabetes related resources and devices. 15. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other Trinity Health and MercyOne policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior. MINIMUM QUALIFICATIONS 1. Must possess a comprehensive knowledge of Nursing, as normally obtained through a Bachelor of Science in Nursing BSN (required), Master’s degree in Nursing preferred, and minimum of 2-3 years of impatient clinical experience. 2. Experience managing diabetes care and insulin therapy in a hospital setting 3. Strong clinical knowledge of diabetes pathophysiology and treatment 4. Strong clinical assessment and critical‑thinking skills 5. Effective communication and ability to work closely with physicians, nursing staff, case management, pharmacy, and other interdisciplinary team members 6. Ability to lead education and change management initiatives 7. Proficiency with electronic health records and clinical software systems 8. Must be comfortable operating in a collaborative, shared leadership environment. 9. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health and MercyOne. Must be able to travel to the various MercyOne/Trinity Health sites (20%) as needed. 3. Must possess the ability to comply with MercyOne and Trinity Health policies and procedures. 4.
