Social Worker - Primary Care at Outpatient Medical Center

The senior social worker provides specialty consultations and psychosocial support to patients and families within a multidisciplinary primary care team. They are responsible for developing effective case management programs, coordinating care for complex patients, and providing instruction to social work students.

Candidates must hold a master of social work degree from an accredited program and possess at least one year of full-Time social work experience in a healthcare setting. Additionally, the role requires obtaining specific washington state social work credentials within 90 days of hire.
