Transition Coordinator - AMIkids Polk

The transition coordinator manages the reentry process for youth, ensuring they have comprehensive service plans and support for their educational and vocational goals. They also provide direct supervision, maintain safety, and facilitate communication between the youth, their families, and external partners.

Candidates must hold a bachelor's degree in counseling, social work, psychology, special education, or a related human services field. A minimum of one year of experience working with youth is required, with experience working with at-Risk youth considered a plus.
