- Summary: The Diabetes Care – Education Specialist provides comprehensive diabetes and prediabetes education to support patients in developing the knowledge, skills, and behaviors needed for effective self-management. This role delivers evidence-based education and ongoing support tailored to each patient’s individual needs, goals, and life experiences. Working collaboratively with an interdisciplinary care team, the Specialist helps improve clinical outcomes, enhance quality of life, and promote sustained self-care behaviors. Through clear communication, patient-centered teaching, and consistent follow-up, this position plays an important role in empowering patients to actively manage their health.
- Key
- Responsibilities: Provide individualized and group diabetes and prediabetes education using evidence-based standards to support informed decision-making and self-management. Assess patient learning needs, readiness, and barriers to develop personalized education plans that support long-term behavior change. Educate patients on nutrition, medication management, glucose monitoring, insulin therapy, and use of diabetes-related technology as appropriate. Collaborate with physicians, nurses, dietitians, and other care team members to coordinate care and reinforce consistent treatment plans. Document education encounters, patient progress, and outcomes accurately within the electronic health record in accordance with organizational standards. Participate in quality improvement initiatives and maintain professional competency through continuing education and certification requirements.

- Required: Current or temporary RN license in ND (or compact RN licensure) required. Diabetes Care and Education Specialist certification required upon hire. Certification in insulin pump training required upon hire.
