Join the Largest Pediatric Rehabilitation Provider in New Mexico MECA Therapies is seeking a compassionate and organized Family Service Coordinator (FSC) to join our Early Intervention team. As the largest pediatric rehabilitation provider in New Mexico, MECA is dedicated to ensuring children and families have access to high-quality services and support within the communities we serve. Our Family Service Coordinators play a critical role in helping families navigate Early Intervention services, coordinate resources, and ensure children receive the support needed to reach their developmental goals. Position Summary

The Family Service Coordinator (FSC) supports families of children birth to three years of age by coordinating Early Intervention services and assisting families in accessing resources, supports, and developmental services. The FSC serves as the primary point of contact for families, helping guide them through the Early Intervention process, supporting Individualized Family Service Plan (IFSP) development, coordinating services, and ensuring services are delivered effectively and timely. This position provides services primarily in home and community settings and requires collaboration with families, therapists, healthcare providers, community partners, and interdisciplinary teams.

Qualifications

Required Bachelor's degree in one of the following areas: Social Work Counseling Psychology Nursing Special Education Early Childhood Education Related field Bachelor's degree in another field may be considered with two (2) years of direct experience serving individuals with disabilities and/or families. Ability to complete required Family Service Coordination training modules within one year of hire. Strong organizational and record-keeping skills. Effective verbal and written communication skills. Strong interpersonal and collaboration skills. Ability to maintain confidentiality and comply with HIPAA requirements. Proficiency with Microsoft Windows and electronic documentation systems. Valid driver's license, reliable transportation, and automobile insurance. Preferred Experience working with children birth to three and their families. Early Intervention experience. Knowledge of Individualized Family Service Plans (IFSPs). Experience coordinating community resources and family services. Bilingual (English/Spanish) preferred. Essential Responsibilities Serve as the primary contact and support for families enrolled in Early Intervention services. Explain the Early Intervention process, family rights, procedural safeguards, and available resources. Gather information regarding family concerns, priorities, resources, and desired outcomes. Coordinate evaluations, assessments, eligibility determination, and service planning activities. Assist families in developing and preparing for Individualized Family Service Plans (IFSPs). Coordinate and monitor Early Intervention services identified on the IFSP. Connect families with community resources, medical providers, and additional supports. Facilitate Transdisciplinary Team Consultation Meetings (TTCMs), IFSP meetings, periodic reviews, annual reviews, and transition activities. Ensure services are delivered in a timely manner and aligned with family priorities and child needs. Support families in promoting development across cognitive, communication, physical/motor, social-emotional, and adaptive domains. Assist families with navigating insurance requirements, including public and private insurance documentation when applicable. Complete required transition planning and support families through transition conferences. Conduct required family visits, including initial face-to-face visits and ongoing monthly face-to-face contact. Maintain strong relationships with families while supporting informed decision-making and family-centered practices. Caseload & Productivity Expectations Maintain an assigned caseload of approximately 38–40 families per month, based on eligibility criteria, service needs, and program requirements. Provide services according to each child's IFSP. Meet established productivity expectations while maintaining high-quality family-centered services. Caseload assignments may be adjusted based on program needs and collaboration with leadership. Documentation & Compliance Maintain accurate and confidential records for each child and family served. Complete documentation required by MECA, Early Childhood Education and Care Department (ECECD), New Mexico FIT Program, and other applicable agencies. Maintain records including: Consent and release forms Evaluations and assessment results Eligibility documentation Medical records IFSP documentation TTCM notes and family contact documentation Document all time spent supporting families and coordinating services. Complete required documentation in the designated data management system. Submit monthly documentation and billing requirements according to program timelines. Participate in file reviews and quality assurance activities. Professional Development Complete required Family Service Coordination training modules within one year of hire. Obtain and maintain required certification through the New Mexico FIT Program. Participate in required MECA trainings, staff meetings, and professional development activities. Provide consultation, mentoring, and training to team members as appropriate. Maintain knowledge of Early Intervention regulations, best practices, and family-centered care approaches. Supervision & Support Family Service Coordinators receive ongoing support and supervision from designated Early Childhood Services leadership, including: Lead Family Service Coordinator (LFSC) Early Childhood Lead Coordinator (ECLC) Assistant Regional Director (ARD) Regional Director Supervision is based on program requirements, experience level, and complexity of assigned responsibilities.
