Description The Physical Therapist will partner with Early Learning programs (serving children birth- age 5) in St. Joseph County to coordinate the needs of early learning programs, provide training and education on delays, disabilities and challenging behaviors to teachers, families, and the community.
The Physical Therapist provides evaluations and treatment in various settings, assessing the gross motor area of development. This therapist will provide treatment plans and outcomes reflected in each client’s authorization in accordance with the payor requirements, using therapeutic modalities and procedures to maintain and increase skills. The Physical Therapist will also communicate with staff and families regarding results of the evaluation, treatment and progress, provides parent training, and makes suggestions for home programming.
At times, additional First Steps services and supports will be required to sustain progress for these children. A First Steps Physical Therapist is responsible for implementing therapeutic support in the home and/or the child’s natural environment. The therapist will plan, implement, evaluate, and document each therapeutic plan to ensure proper programming in conjunction with the child's parent/guardian during home visits.

Essential Functions
· Assist in evaluating persons served with other therapists or individually. · Communicate the results of the assessment/screening as directed. · Determine each client’s adaptive equipment/orthopedic equipment and/or assistive technology needs. This includes specially adapted chairs, and wheelchair inserts for proper positioning orthotics, splints, braces, walkers, etc.
· Collaborates with the family and provides individual therapy when appropriate to maintain and improve gross motor development; muscle strength and ranges of motion, positioning, expanding mobility options, using equipment effectively, facilitating safety for the home and community and providing information on the client’s physical and healthcare needs · Provide and/or obtains the necessary equipment for proper positioning and play if applicable.
· Responsible for the health and safety of the people served. · Expected to serve as an advocate to protect the rights of persons served. · Expected to participate in training as required by their supervisor or for all LOGAN staff. · Expected to perform other duties as assigned by their supervisor. · Plan instructional activities for clients and assist families in accessing community activities. · Furnish all required monthly statistics on a timely basis. · Respect the confidentiality of persons served and will respect their cultural diversity, in accordance with LOGAN policies. · May be asked to serve on a LOGAN committee that addresses the internal operations of the agency. · Other duties as deemed necessary for the betterment of the program.
Qualifications: Education · A registered and licensed Physical Therapist in the State of Indiana
Knowledge, Skills, and Abilities · Communicate respectfully and appropriately with staff, supervisors, and agency personnel. · Work collaboratively and respectfully with other therapeutic disciplines · Establish and maintain effective relationships with persons served and family members/guardians. · Able to be creative in exploring new and innovative ideas. · Ability to work independently and use sound judgment · Currently registered/licensed with the Health Professions Bureau of Indiana. · INDIANA ONLY: Have a valid driver’s license and ability to drive to and from each visit, therapy session, or Individual Family Service Plan site. · Have a reliable form of transportation. · Indiana Only: Enrolled in the state of Indiana as First Steps provider. · Ability to lift 50 pounds throughout a session
