Case Manager - Foster Care (KY)

The case manager supports children and foster parents by developing treatment plans and coordinating essential medical, therapy, and educational services. They are responsible for placement assessments, ongoing monitoring of foster home compliance, and managing crisis situations through an on-Call rotation.

Candidates must be at least 21 years old with a bachelor's degree in human services or a professional license (Lsw/Lpc) and at least two years of experience working with children with emotional or behavioral problems. A valid driver's license and specific auto insurance coverage are required for travel between client locations.
